Since the State of Illinois has no common sense, this form is required to be sent to every registered voter. The State thinks people aren’t smart enough to figure out how to request a mail in ballot themselves.

The Bloomington Election Commission will be sending them too.

If you get one of these for someone who doesn’t live at your address:

They are registered to vote at your address and need to be removed!

Contact whoever your election authority is. You shouldn’t have to mail it back to them – but this is government so . . . . .
